All dogs adopted from NOT SO GRANDE Dachshund and Small Dog Rescue are or will be spayed or neutered, have, or will be given current shots for their age, and are micro chipped. By adopting a dog and by signing this form, the adopting party agrees to comply with all provisions of this agreement.

Unfortunately, an animal that appears healthy when adopted may, despite our best efforts, become ill. If your new pet stops eating, is listless, vomits, has a diarrhea, or exhibits other disease symptoms, take him/her to your veterinarian immediately.

I agree the dog I am adopting appears to be in good health and that I am adopting the animal in as is condition. I agree that I will not & cannot hold Not so Grande Rescue liable for any medical bills,veterinarian treatment, ECT... and have no right to slander defame of file a complaint with any state federal or local organization to slander defame or extort any adoption fees or bills incurred by me. I also agree I will be held liable for any negative result of my action should I brake my agreement and agree I will pay all damages to Not so Grande Dachshund & Small Dog Rescue.

I understand that NOT SO GRANDE Dachshund and Small Dog Rescue cannot and does not make any representations of warranties, either expressed or implied concerning the temperament, habits, health, pedigree, disposition, age, or background of this dog and that I have no right to a reimbursement of medical, professional bill's or adoption fees.

I understand that Not So Grande Dachshund & Small Dog Rescue does not refund adoption fees, or any medical bills paid to the organization or veterinarian. That any money paid will be used for the rescue and will be considered a Donation! if I must return the animal I adopted from NSG there will be no refund of expenses, adoption fees or any medical bills.

Furthermore, I understand that the animals future reactions to me, my family or any these persons are completely unpredictable because animals, like people, have their own personalities! Therefor Not so Grande Dachshund & Small Dog Rescue, there employees, volunteers,board members, will not and cannot be held liable for any negative reaction, injury, biting scratching, nipping at the time of appointment or after adoption. And that I inter the property and meeting place for the adoption at my own risk and free will.

NOT SO GRANDE Dachshund and Small Dog Rescue reserves the right to conduct premise inspections (Home visits) before and after the adoption of the dog.

I assume full responsibility to provide proper food, clean water, shelter, veterinary care, exercise, and a safe environment for this pet. I am aware that small dogs need extra provisions such as sweaters, blankets, harness, ramp to enter or exit bed or couch, regular grooming, and mainly need to live indoors out of the elements. Also, small dogs are more likely than big dogs to need dental work later in life.

In consideration of receiving this pet the adopting party hereby releases NOT SO GRANDE Dachshund and Small Dog Rescue, it's board members, officers, volunteers, agents, and employees from all claims of injury or damage which this dog may cause the adopter or the adopter's property. The adopting party further agrees to hold harmless, defend and indemnify NOT SO GRANDE Dachshund and Small Dog Rescue, it's board members, officers, volunteers, agents, and employees from all claims of liability to other persons for injuries or damage caused to them or their property by this dog adopted by the adopting party.

If for any reason, the adopting party does not comply with all provisions of this agreement, the adopting party agrees to return the adopted dog to NOT SO GRANDE Dachshund and Small Dog Rescue upon demand of NSG. I agree, that if for any reason, throughout the dog's lifetime, if I cannot keep the new dog I understand that I must return the dog to NOT SO GRANDE.

My signature below attests that I have read and understand this agreement. The information provided in this application (in its entirety) is true to the best of my knowledge as of the date on this application.
